School-Based Health Center at EastSide Charter School

Westside Family Healthcare’s School-Based Health Center (SBHC) at EastSide Charter School makes it easier for students to access healthcare right where they learn.

Located on campus and staffed by Westside Family Healthcare providers, the SBHC allows students to receive care during the school day without leaving school.

The SBHC does not replace your child’s regular primary care provider. Our team works with your child’s healthcare provider to coordinate care and support your child’s overall health.

What Parents Should Know

  • Parent or guardian consent is required. Written permission must be provided before your child can receive services.
  • Student confidentiality is protected. School and medical records are kept separate.
  • Insurance is not required. Services are available regardless of insurance status.
  • There is no copay for services provided at the SBHC.

How to Enroll

To enroll your child in the SBHC, please complete and return the following:

  • Consent form, student registration form and health history form
  • Up-to-date insurance information (if your child is insured)
  • Completed enrollment forms should be returned to the SBHC.
