Rural Health Outreach

Our mobile health unit (MHU) brings primary care directly to farms and rural housing communities, typically in the evenings after work hours. The outreach team includes providers, nurses and community health workers who focus on prevention, early detection and helping patients manage illness.

In addition to our mobile health unit, we also visit farms and provide transportation to Westside health centers for patients who need in-office services, ensuring access to a full range of care.

MHU services include:

• Health screenings and education
• Preventive care (physicals, vaccines)
• Chronic disease management (high blood pressure, diabetes)
• Sick visits for illness or injury
• Access to resources for social services
• Assistance with transportation for visits
• Supplies (hygiene kits, care kits, masks, food)

Westside partners with local community organizations and stakeholders through our Rural Health Outreach Advisory Council to ensure our services meet the needs of the farmworker community.

See a Westside provider from the comfort of home using a secure video visit. Telehealth is available for many primary care needs, including follow-ups, sick visits and medication management.
