Please fill out the online patient registration forms sent to you by email or text before your appointment.
*What if I don’t have a health insurance card?
Not to worry! Westside has a sliding-fee scale, or a representative can assist you with health insurance enrollment. In this case, bring one of the approved proof-of-income documents:
Four consecutive pay stubs
W-2 statement
Bank statement
Most recent tax return
Telehealth Appointments
If you have a telehealth appointment, it is helpful to set up a Westside patient portal account before your telehealth appointment. Your provider can easily follow up afterward with lab results and other information you might need.
