In-Person Appointments

When coming in for an in-person appointment, please bring:

  • Photo ID (driver's license, state ID, passport)

  • List of current medications (prescribed or over the counter) and supplements (vitamins, etc.)

  • Past health records (past or present problems, surgeries, immunizations, etc.)

  • Family health history (any health information you know about family members)

  • Health insurance card*

  • Co-payment

Please fill out the online patient registration forms sent to you by email or text before your appointment.

*What if I don’t have a health insurance card?

Not to worry! Westside has a sliding-fee scale, or a representative can assist you with health insurance enrollment. In this case, bring one of the approved proof-of-income documents:

  • Four consecutive pay stubs

  • W-2 statement

  • Bank statement

  • Most recent tax return

Telehealth Appointments

  • If you have a telehealth appointment, it is helpful to set up a Westside patient portal account before your telehealth appointment. Your provider can easily follow up afterward with lab results and other information you might need.

Financial Assistance and Insurance

As with our in-person visits, there is a cost for telehealth visits. We accept most insurance plans and have a sliding-fee scale, based on income and family size, to help make your care more affordable.
